What we need to know

Most vision insurance plans cover a certain maximum dollar amount per year. We'll help you maximize the use of your vision care benefits here at Triangle Eyecare Optometry Long Beach. To help you, we will need to know the following:

• name of vision plan subscriber
• subscriber's insurance ID number
• birthdate of subscriber
• social security number of subscriber is needed for some plans
• the 800 phone number for your plan (for those not listed above)

Payment Options for Fees Not Covered

Please review your insurance benefit details as any fees not covered by your insurance plan are your responsibty. For your convenience we do accept cash, checks, debit cards, Visa, MasterCard, and Discover.
