Insurance card or information, including the name of your health or vision plan, your ID#, and the 800-number for the plan to check your eligility.
Complete list of any medications you are taking
Read the privacy notice and be ready to sign a form in the office

Triangle Eyecare Optometry Long Beach is at the corner of Carson St & Bellflower Blvd in Long Beach. If you're driving west on Carson, turn right at the 2nd driveway and into our parking lot. From Carson driving east or from Bellflower, turn on Viking Way. There's plenty of free parking right next to our office in our private parking lot as well as free street parking along Viking Way right in front of the office.
